পাইলস, যাকে অর্শ্বরোগও বলা হয়। বৃহদান্ত্রের শেষাংশে রেকটামের ভেতরে ও বাইরে থাকা কুশনের মতো একটি রক্তশিরার জালিকা থাকে, যা প্রয়োজন সাপেক্ষে সংকুচিত ও প্রসারিত হয় যা আমরা পাইলস নামে জেনে থাকি। যখন পায়ুপথে এসব শিরার সংক্রমণ বা প্রদাহ হয় এবং চাপ পড়ে তখন পাইলস বা হেমোরয়েডসে প্রদাহ হয়। যাকে সাধারণ ভাষায় অর্শরোগ বলা হয়।

এটি মলদ্বারের এক ধরনের জটিল রোগ। পাইলস এর ফলে রক্তনালিগুলো বড় হয়ে গিয়ে ভাসকুলার কুশন তৈরি হয়। শিশু থেকে বৃদ্ধ যেকোন বয়সের মানুষ এ জটিল রোগে আক্রান্ত হতে পারে। পাইলস হলে সাধারণত চুলকানি বা রক্তক্ষরণ হয়। মলদ্বারের নিচের অংশে গোল আকারে ফুলে উঠে, ফলে যে কোন সময় সেই জায়গা থেকে রক্তপাত হতে থাকে। এটি খুবই অস্বস্তিকর এবং যন্ত্রনাদায়ক।

দীর্ঘকালীন কোষ্ঠকাঠিন্যের রোগীদের পাইলসের সমস্যা দেখা দেওয়া অস্বাভাবিক কিছু নয়। এছাড়া যাদের মলত্যাগের অকারণে বেগ প্রদানের বদভ্যাস রয়েছে, তারাও এই রোগ বাধিয়ে ফেলতে পারেন।

পাইলসের প্রকারভেদঃ

সাধারণত পাইলস দুই প্রকার হয়ে থাকে -

১. অভ্যন্তরীণ পাইলস

২. বাহ্যিক পাইলস

অভ্যন্তরীণ পাইলস এবং বাহ্যিক পাইলস মলদ্বারে তাদের অবস্থানের উপর ভিত্তি করে। এগুলি সাধারণ এবং মলদ্বারের ভিতরে মলদ্বারের খোলার উপরে ২ থেকে ৪ সেন্টিমিটার (সেমি) এর মধ্যে ঘটে।

১. অভ্যন্তরীণ পাইলস:

অভ্যন্তরীণ অর্শ্বরোগ চারটি শ্রেণী বা পর্যায়ের হয় যা প্রোল্যাপের উপর ভিত্তি করে।

প্রথম পর্যায় - পাইলস ফুলে বাইরের দিকে বের হয়ে আসে না বা প্রলেপস হয় না।

দ্বিতীয় পর্যায় - মলমূত্র ত্যাগের পর পাইলস ফুলে বাইরের দিকে বের হয়ে আসে এবং তারপর আপনা-আপনি ঠিক হয়ে যায়।

তৃতীয় পর্যায় - পাইলস ফুলে বাইরের দিকে বের হয়ে আসে এবং নিজে নিজে ঠিক করতে হয়।

চতুর্থ পযার্য় - পাইলস ফুলে বাইরের দিকে বের হয়ে আসে বা প্রলেপস হয় এবং তা আর নিজে ঠিক হয় না বা করা যায় না।

২. বাহ্যিক পাইলস:

বাহ্যিক পাইলস মলদ্বারের বাইরের প্রান্তে ছোট ছোট গলদ গঠন করে। এগুলো প্রায়শই চুলকানিদায়ক এবং বেদনাদায়ক হয়ে থাকে।

পাইলসের কারণ -

পাইলসের প্রধান কারণগুলো হচ্ছে দীর্ঘদিন কোষ্ঠকাঠিণ্যে ভোগা, পুরনো ডায়রিয়া, মলত্যাগে দীর্ঘক্ষণ টয়লেটে বসে থাকা ও দীর্ঘক্ষণ দাঁড়িয়ে থাকা। এছাড়া পারিবারিক ইতিহাস, আঁশযুক্ত খাবার কম খাওয়া, ভারি মালপত্র বহন করা, স্থুলতা, কায়িক শ্রম কম করা।

গর্ভকালীন সময়ে, পায়ুপথে যৌনক্রিয়া, যকৃত রোগ বা লিভার সিরোসিস ইত্যাদি কারণেও এ রোগের আশংকা বেড়ে যায়। সর্বোপরি পোর্টাল ভেনাস সিস্টেমে কোনো ভাল্ব না থাকায় উপরিউক্ত যে কোনো কারণে পায়ু অঞ্চলে শিরাগুলোতে চাপের ফলে পাইলস সৃষ্টি হয়।

পাইলসের লক্ষণ -

পাইলস রোগে যেসব লক্ষণ দেখা যায় তা হচ্ছে- পায়ুপথের অন্ত্র বা ভেতরের পাইলস রোগে সাধারণত তেমন কোনো ব্যথা বেদনা, অস্বস্তি থাকে না। অন্যদিকে পায়ুপথের বহিঃঅর্শরোগে পায়ুপথ চুলকায়, বসলে ব্যথা করে, পায়খানার সঙ্গে টকটকে লাল রক্ত দেখা যায় বা শৌচ করার টিস্যুতে তাজা রক্ত লেগে থাকে, মলত্যাগে ব্যথা লাগা, পায়ুর চারপাশে এক বা একের অধিক থোকা থোকা ফোলা থাকে।

পাইলসের সাধারণ লক্ষণগুলি হলঃ

• পায়ূ অঞ্চলে ব্যথা এবং চুলকানি।

• মল বা মলত্যাগের পর রক্ত।

• মলদ্বারের চারপাশে একটি শক্ত গলদা।

• অভ্যন্তরীণ অর্শ্বরোগের লক্ষণ:

• মল অতিক্রম করার সময় অতিরিক্ত চাপ বা জ্বালা হতে পারে।

• মলত্যাগের সময় ব্যথাহীন রক্তপাত।

• যদি পাইলস প্রল্যাপস, ব্যথা এবং জ্বালা হয়।

• বহিরাগত অর্শ্বরোগের লক্ষণগুলির মধ্যে রয়েছে:

• মলদ্বারের চারপাশে চুলকানি।

• মলদ্বারের কাছে বেদনাদায়ক মাংসল গলদ।

• বসার সময় ব্যথা বা অস্বস্তি।

Homeopathic Medicines for Piles.

1. Aesculus Hippocastanum – For Painful, External, Non-bleeding Piles

It is the best and most frequently used medicine for painful piles. It is taken when pain is felt during or after passing stool. The pain continues for long hours after the passing of stool. Pain is very sharp that may extend from the anus to the lower back. A person may have a distinct feeling of small sticks filling up the rectum. It is a specific medicine for external piles and blind piles (non-bleeding piles). This medicine does wonders on engorged haemorrhoidal veins, brings great relief in pain and also helps in shrinking piles naturally.

Dosage: The most commonly recommended dosage, one may use 30C potency two to three times a day as per the intensity of the symptoms.

2. Nux Vomica – For Piles With Constipation And Ineffective Urge To Pass Stool

Nux Vomica is the best medicine to treat piles with constipation and ineffective urge to pass the stool.

Those who need this medicine have to go pass stool several times in a day to completely clear the bowels. They pass stool bit by bit throughout the day and yet have the urge to pass the stool. They also have marked pain and itching at the anus. Nux Vomica is recommended for piles that is non-bleeding.

For piles or constipation triggered by alcohol intake, Nux Vomica is the right medicine. It is also the best choice of medicine for people with piles who lead a sedentary lifestyle.

Dosage: Though it can be used in both low to high potencies, in the beginning, it is advised to take it in 30C potency twice or thrice a day.

3. Aloe Socotrina – For Protruding Piles

Hamamelis is useful in cases where piles is internal, painless and it bleeds. ‘Painless, non bleeding piles’ these four words are an indication to describe the use of this medicine in its entirety, and it requires no further description.

Dosage: One may take Hamamelis in 30C potency three to four times a day as per the intensity of the problem.

5. Collinsonia Canadensis – For Piles With Constipation (Dry, Hard Stool)

এই সমস্যার প্রধান উপসর্গ তলপেটজুড়ে ব্যথা। কিশোরী বয়সে মাসিকের শুরু থেকেই দেখা দিতে পারে এন্ডোমেট্রিওসিসের ব্যথা, দুর্ভাগ্যজনকভাবে চলতে পারে মেনোপজ পর্যন্ত। নারীদের জরায়ুতে যে এন্ডোমেট্রিয়াল লাইন থাকে, তার কোষ জরায়ুর বাইরে ফেলোপিয়ান টিউব, ডিম্বাশয় বা পাউচ অব ডগলাসে লেপ্টে বসে থাকলে তাকে এন্ডোমেট্রিওসিস বলে। ঋতুস্রাবের আগে হরমোনের প্রভাবে এই সব অস্বাভাবিক এন্ডোমেট্রিয়াম টিস্যুগুলোও ছিঁড়ে বেরিয়ে আসে এবং প্রচুর রক্তপাত হয়। আর এ কারণে পেটে ভয়ানক ব্যথা হয়। কখনো কখনো রেক্টাম বা মলাশয়েও এটি হতে পারে।

অনেকের আবার অল্প বিস্তর এন্ডোমেট্রিওসিস থাকলেও কোনো উপসর্গই থাকে না। বন্ধ্যত্ব বা অন্য কারণে পরীক্ষা করতে গিয়ে ধরা পড়ে।

 প্রথম বা দ্বিতীয় পর্যায়ে বেশির ভাগ সময়ে খুব একটা সমস্যা হয় না। কিন্তু তৃতীয় বা চতুর্থ পর্যায়ে পৌঁছালে স্বাভাবিকের তুলনায় অতিরিক্ত বেশি রক্তপাত হয়। কিছুটা রক্ত পেটের মধ্যে জমা থেকে যায়। আর সমস্যা হয় এর থেকেই। জমা রক্ত চকলেট সিস্ট হয়ে পিরিয়ডের সময় এবং কখনো কখনো সারাক্ষণই পেটে ব্যথা করতে থাকে।

 মাসিকের কিছুদিন আগে থেকে তলপেট খুব ব্যথা করে, মাসিক চলাকালীন ব্যথা বাড়তে থাকে এবং মাসিকের শেষ দিকে ব্যথা তীব্র হয়। হেভি মেন্সট্যুয়াল ব্লিডিং বা তীব্র রক্তপাত হতে পারে।

 প্রস্রাব বা মলত্যাগের সময়ও মেয়েরা ব্যথায় কাতর হয়ে পড়ে। বিবাহিত মেয়েদের যৌন সংসর্গের সময় মারাত্মক ব্যথা ও যন্ত্রণা হয়। তলপেট ছাড়াও কোমরে ব্যথা করে।
